Business Documentation
Every company must provide copies of the documents listed below to complete the program. Documentation requirements serve to establish that the company is operating legally and ethically.
It is suggested that these documents be submitted prior to the Accreditation application.
Business Plan Documents
- Mission statement
This is a short, written statement of your company’s purpose. - 5-year business plan
This is the main document that you use to gauge your company’s success, identify competition, and plan the way ahead. - Sample budget
This is a listing of your projected income and expenses, organized by categories over the next 12 months. Samples are readily available by searching online. - Cash flow projection
The cash flow projection shows how cash is expected to flow in and out of your business. This helps you determine how much capital investment your business needs.
Business Operations Documents
- Federal tax identification number
This number is issued by the Internal Revenue Service. - State tax identification number
This number is issued by your state Department of Revenue. - Business license
If you are required by your town or city to have a business license, then we need a copy. - Contractor license
If you are required by your town or city to have a contractor license, then we need a copy. - EPA registration
This is issued by the EPA after attending an official training seminar.
If your company does not provide lead services, then this is not required. - Proof of insurance
Insurance companies issue proof of insurance cards for your car, general liability certificates, and proof of workers’ compensation. If you have any of these, then we need copies. - Estimating procedures
This is a short description of the procedures you use to give an estimate before a project. - Sample business proposal
This is a copy of the paperwork that you would give a prospective customer for their approval before a project. - Safety plan
This is a written plan detailing your company’s policies on safety, rules, hazard communication program details, respiratory protection, lead compliance, protection from falls, and a log of safety meetings.
Accreditation Application Documents
- Third party references
We require two business references. They can be from customers, suppliers and other vendors, etc. - Code of Ethics
The application process requires that you sign a copy of the Code of Ethics to ensure your understanding of the trade’s ethical requirements.
